If someone is going to put a spoiler up on purpose, they're not going to bother putting anything in spoiler tags anyway. There could be a workaround to it, somewhat similar to the removal of IMG tags in quotes, I just don't have the time right now to look into it.

If you go through someone's posts and on the second, third etc. page, if there is a youtube tag and you click on it, it goes back to the first page of their posts and you have to go to the actual thread to view it
