I can't take my eyes off the bandage on Mac's cheek. Is it just a sign of Screen-KO or has battle damage just been this well hidden from us?

I was actually thinking they were going to bring in this beta element of Brawl:  Hence the bruise and bandage on Little Mac. Mario was most likely ko'd at a low percent. I am most likely wrong, let's just wait and see. _________________      |

visual changes depending on damage have already been shown
the characters start giving off steam at high percentages, remember? that'll just be something funny in little mac's screen ko |

I don't think we'll get full out battle damage for every character, but considering that it was actually present in Punch-Out, I can see Sakurai adding it for Little Mac to make the character more authentic. Who knows.
